If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. We can verify insurance coverage and benefits before your appointments by emailing the forms back to us ahead of time. Thank you and please contact our office if you have any questions at all.
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Insurance payments are estimated on treatment plans. Patient payment is expected at the time services are performed. Feel free to contact our office and speak with Rachel if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements. A 5 % discount is offered for payment in full at time of service. We accept Care Credit and patients can apply online for approval prior to appointment.
